I am currently in Network with Aetna, Regence Blue Cross/Blue Shield, MODA, and Pacific Source. Please note that you may be responsible for copays or deductibles - please refer to your plan documents for details. I cannot see Medicare or OHP/Medicaid patients at this time.

For other insurance plans, including Oregon Health Plan (OHP), I am out of network. Please contact me for information regarding out of pocket fees - I offer sliding scale pricing for patients who are in need of my services. For out of network patients, I offer a superbill that you can submit to your insurance for partial reimbursement.
